Please bring these two things with you – or mail them – before your first session:

1. The completed Client Information Form

2. The signed signature page from my Disclosure Statement (Page 4)

Please read the Disclosure Statement, Privacy Practices, and Telehealth Policy documents. You can keep them on your computer for future reference, or you can print them out.
